RMA Request

To initiate the return of equipment or parts to JIREH for repair, please complete the following form to request an RMA number. You will receive your RMA number shortly after submitting the form.

Please include the RMA number on the shipment along with any communication with JIREH. Please DO NOT send your equipment or parts to JIREH without obtaining an RMA number. We cannot process equipment or receive parts without an RMA number. In such cases, they will be returned to the sender at their expense.

Please note that repairs are processed in the order they arrive at JIREH's facility.

Motorized systems containing magnets may be classified as dangerous goods unless shipped in their original shielded carrying cases.*

Lithium-ion batteries are classified as dangerous goods. Any lithium-ion batteries have been removed from the shipment are not included with the equipment being returned for repair.*

All equipment is clean, dry, and free of contamination.*

Any equipment exposed to biohazardous materials has been properly cleaned and sanitized before shipment.*

Add Rush Repair Service ($150USD expediting fee and 40% labor surcharge).

Repair Authorization*

Proceed with repair, no quotation required. I authorize JIREH to proceed with the repair without prior quotation approval.

Provide a quotation before repair. Do not proceed with the repair until I review and approve the quotation. A $100 USD service fee applies and may extend repair time.
